April 9th is for Youth Only Physicals and will be conducted by the UCF Nurse training program with coverage by doctors and physician assistants who legally can sign off on the physical.
Please read the NOTES so you will come with the appropriate paperwork.

The troop is required to have BSA Medical Forms for each participant (youth/adult) who is going camping with the troop.
The BSA Medical Form has 3 Parts – A, B and C. A and B are required to go on weekend camping trips. Part C (doctor sign-off) is required for Summer Camps, High Adventure, etc.

NOTE: we do need to have vaccination records. You can bring a copy of the record provided to your child’s school OR request that your pediatrician print a duplicate sheet. There is no need to transcribe all these records because the printed sheet is completely acceptable.
Adults are required to have all 3 parts to attend summer camps/high adventure.
Note: Philmont, Northern Tier and Seabase all have an extra page with specific risks to be aware of for those specific trips.
